- The revised video gaming license Application, Personal History Questionnaire,
Personal Financial Questionnaire, and release forms have been printed
and will be available for use on April 21, 2003. The application instructions
also have been revised. Copies of the revised forms may be obtained
from our Baton Rouge office, any field office, or our web site at www.lsp.org.
One notable revision is that applicants for restaurant and lounge licenses
will no longer have to submit a Personal Financial Questionnaire unless
requested by an investigator. This is applicable only if the applicant
submits an application on the revised forms. The use of the revised
forms will be optional until June 30, 2003. After June 30, 2003, only
the revised forms will be accepted for new applications.
- A new rule concerning the submission of renewal applications has been
promulgated and is posted in the March 2003 Louisiana Register. The
rule, L.A.C. 42:XI.2405.B(3)(c) states:
If a licensee fails to file a complete renewal application
on or before forty five days prior to the license expiration date,
the division may assess a civil penalty of $250 for the first violation,
$500 for the second violation and $1000 for the third violation.
A copy of the rule may be obtained from the Office of the State Register
or located at their web site, http://www.state.la.us/osr/osr.htm.
- It has come to our attention that some device owners may have been
solicited by an unlicensed entity interested in selling video draw poker
device parts. Please be reminded that parts for video draw poker devices
may only be purchased from businesses licensed as a video draw poker
distributor.
